I
have explained to the patient and wife the procedure of bilateral vasectomy,
its intended objectives, and possible complications including but not limited
to recanalization, infection, hematoma, spermatocytic granuloma, and the
possible correlation between vasectomy and arteriosclerotic disease. No guarantee of permanency has been
given. They claim they understand and
accept and will return p.r.n.
